The research

Sorting what we know from what we wonder.

Sorting what we know from what we wonder.

Sorting what we know from what we wonder.

A formal literature review is in preparation. It sorts the research honestly into three layers: what is established, what is a reasonable extension, and what is still an open question.

Where the evidence is early, we say so. Where it comes from animal studies or a single case, we say that too.

Why sort it this way

Sorting what we know from what we wonder.

Sorting what we know from what we wonder.

Wisdom begins in wonder. Every citation is checked against its source before it is shared, and an open question is named as an open question rather than presented as a finding.

What does the research say about ALF?

A fair question deserves an honest answer

If you're asking for the research on ALF, thank you. That's exactly the right question to ask about any treatment, including ours.

The research written specifically about the ALF appliance is limited. What has been published is mostly articles describing the appliance and reports of individual patients treated by clinicians. Those are useful for understanding how the appliance has been used. They aren't controlled studies, and they can't tell anyone how often a result will happen.

Much of that writing also explains ALF through claims about reshaping the skull and correcting skeletal structure. NeuralFORM doesn't make those claims. The articles are listed below so you can read them for yourself.

What we kept, and what we left behind

Kept: light, intermittent forces that work with the body rather than against it. The tongue as the original expander, with the appliance there to support tongue function, not replace it. Function first, assessed at every visit.

Left behind: claims that the appliance corrects the skull or reshapes bone on its own, and claims of cure. NeuralFORM doesn't claim skeletal change from the device, and our appliances aren't claimed to treat, cure or prevent any disease or condition.

For clinicians: where the evidence sits

Animal research shows sutures respond more favorably to light, intermittent forces than to static force, and imaging studies show that bone filling in after rapid expansion starts out less dense and matures slowly. From those findings, we infer that light-force approaches may produce better-quality bone. That inference hasn't yet been tested head to head.

Any improvement reported in growing children has to be weighed against natural growth and spontaneous change, and we build that into how we read our own outcomes.

Structure tells you what's possible. Function tells you what is probable.

A recent report on complications

In January 2026, the Journal of Clinical Orthodontics published a report on complications in adult patients treated with ALF, including cases where the appliance was used beyond its intended purpose.

A design applied outside its intended purpose is one of the most common sources of both poor outcomes and fair criticism. That's why our defined scope of use for Synergy ALF: The Trauma Release Design is public, so any clinician can check a practice against it.
